Required NYS Health Exam and Immunizations

All parents and guardians are encouraged to make appointments for their children with their PCP to have their annual health exams and immunizations. After completing the physical exam and immunizations, please submit the documentation to the School Health office.

Physicals

A physical exam is legally required for admission to school in New York State. New York State Education Law requires all newly enrolled students (new entrants) to submit a health certificate within 30 days of starting school.

NYSED requires a physical exam for new entrants and students in Grades Pre-K or K, 1, 3, 5, 7, 9 & 11; annually for interscholastic sports; and working papers as needed; or as required by the Committee on Special Education (CSE) or Committee on Pre-School Special Education (CPSE).

Immunizations

General Rule: Proof of vaccination must be submitted within 14 days of starting attendance.

Transfer Students / Extensions: If a child is transferring from out-of-state or another district, a 30-day grace period MAY be granted if the parent can show a good-faith effort to obtain the records.

Homeless/Temporary Housing: Students in temporary housing are entitled to immediate enrollment even without records, while the district works to track them down.

School-Based Health Centers

Health exams and immunizations are only available for students who are enrolled and registered at the school-based health centers at MVHS and Edward Williams School. The students who do not attend MVHS and the Williams cannot register at the school-based health centers. All parents and guardians should make appointments for their children to have their annual health exam and immunization prior to school starting.
