Please provide a current physical exam from your child's doctor with an updated immunization record when your child enters the following grades:

• Pre-Kindergarten
• Kindergarten
• 2nd Grade
• 4th Grade
• 7th Grade
• 10th Grade

All of the Required immunizations must be documented by your health care provider, health department where the child received the immunizations, or must be from an official copy of the immunization record from the child's previous school (a copy of the original immunization record from the healthcare provider – not a copy of the school health record). A NYSIIS/NYCIR record is also acceptable.

Tdap Immunization Requirement for all 11 Year Old Students:

New York State Department of Health Public Health Law 2164 requires all students who are 11 years old and are entering 6th grade or a comparable age-level special education program to receive an immunization containing Tetanus toxoids, Diphtheria, and Acellular Pertussis (Tdap).


*Ten year old students entering 6th grade will not be required to receive a Tdap vaccine until they turn 11 years of age.*
